-Please call our office to provide the staff with your basic information.
-Our office will send you an electronic New Client Form to fill out. Please sign to agree to our terms and services.
-Once you fill out the form, please call our office back. There is a $1.00 activation fee to finalize the set up. This step further establishes a secure way for us to maintain credit card information.
-From there we can schedule you an appointment based on our soonest availability.
